Mullaney’s, A Guardian Pharmacy (Guardian Pharmacy of Cincinnati LLC) (“Mullaney’s”) is required by regulation to provide this notification.

On March 7, 2022, Mullaney’s learned that, on that same day, an email containing certain patients’ information was inadvertently sent to the personal representative of one patient. The recipient promptly notified Mullaney’s and confirmed the permanent deletion of the email and attachment. As such, information was not released to the public.

The email attachment contained patients’ names, billing addresses, and medication information. It did not contain Social Security numbers, dates of birth, health insurance information, or financial account information.

In addition to providing notice of this email error to affected individuals, Mullaney’s has notified the U.S. Department of Health and Human Services, as required by regulation.

While the company has no reason to believe that there will be any misuse of this information, Mullaney’s takes the security of its patients’ information very seriously. As a precautionary measure, Mullaney’s encourages potentially affected individuals to monitor against incidents of identity theft by reviewing account statements, health insurance account records, and explanation of benefits forms for unrecognized activity, and to promptly report all unknown activity to the institution that issued the record.
